A transition coordinator works with high school special education students to help prepare them for leaving school and living life as an adult. This includes helping students decide where they are going to live, explore job opportunities, and participate in the community. The coordinator works closely with high school teachers, guidance counselors, higher education, local employers and service agencies to assist with transition programming. The transition coordinator is required to have a bachelor's degree and a minimum of three years of satisfactory teaching experience in special or vocational education at the high school level, or a minimum of three years of satisfactory employment providing transition-related service to individuals with disabilities between the ages of 13 to 26 years.
